Our innovative approach centers on access without barriers—including a drop-in Health & Wellness Lounge for unhoused individuals, same-day appointments, wraparound care navigation, and culturally responsive outreach.

What makes Omni different is our integrated care ecosystem: medical providers, therapists, community health workers (CHWs), substance use specialists, and chronic disease coordinators work together across a shared care plan. We go beyond clinical treatment—offering housing referrals, HCV/HBV reintegration support, MAT/MOUD services, STI/HIV education, mental health care, and preventative screenings—meeting people exactly where they are.
